Plugin for shape / questionnaire then generate customized document?

Hello,

I am searching for a plugin that can lend a hand me succeed in the next:

* At the entrance finish, it is a detailed shape with a couple of pages, fields (calculated, dependent, and many others.)
* Then, at the again finish, in line with the consumer’s enter from the shape, it’s going to robotically generate a customized document (that I will arrange) and can ship it by way of e-mail to the consumer

Random Instance: if consumer solutions Sure to Query 1 and a bunch upper than X to Query 4, then a customized sentence shall be added within the document (referencing the values entered within the shape).

Does one thing like that even exist? No longer certain what the most efficient method to construct one thing like this could be.
